We're looking for a full-time Physical Therapist to serve patients in Sonoma County. This individual would be based out of their home while traveling to and from patient homes for clinical care. Key Responsibilities Evaluate patients' functional status, muscle strength, mobility, gait, and ROM to develop and direct individualized treatment plans Instruct patients, families, and caregivers on therapeutic appliances, PT goals, and relevant care phases Monitor and report patient progress and condition changes to the physician Prepare and submit clinical progress summaries and participate in discharge planning Identify needs for additional home health services and refer accordingly Supervise Physical Therapy Assistants and, when PT is the sole skilled service, evaluate home health aide care every two weeks Provide consultation, staff in-services, and participate in peer review as needed Qualifications Bachelor's or Master's degree in Physical Therapy from an accredited program Active state PT license Minimum 2 years of PT experience (home health/community experience preferred) Strong verbal and written communication skills Current CPR certification Valid driver's license, insured vehicle in good working order #J-18808-Ljbffr Best Home Health Providers
