Key Responsibilities: Respond to afterhours crisis calls with urgency Perform timely in-person patient visits Evaluate and document patient needs and responses Assess patient eligibility for hospice services Provide care in varied home environments Qualifications & Skills: Current unrestricted RN licensure in the practice state Valid driver’s license, insurance, and reliable transportation or public transit access Preferred: CPR certification, 1+ years clinical experience, hospice care experience Additional Details: Compensation varies based on experience and location.
