Physical Therapist (PT) – Float Position

Nexus at Wood River | Godfrey | Alton, IL

Job Summary

Nexus is seeking a compassionate and dedicated Physical Therapist (PT) for a Float Position supporting our rehabilitation teams in the Wood River, Godfrey, and Alton areas .

This is an excellent opportunity for a PT who enjoys variety, collaboration, and making a meaningful difference across multiple skilled nursing and rehabilitation settings. The Float PT will provide high-quality, patient-centered physical therapy services based on the needs of each facility.

The ideal candidate is flexible, dependable, clinically confident, and comfortable working with residents across different rehabilitation environments.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services in accordance with physician orders and established treatment plans.
• Provide therapeutic exercises, gait training, balance activities, transfer training, and functional mobility interventions.
• Help residents improve strength, endurance, mobility, balance, and overall functional independence.
• Monitor resident progress and modify treatment plans as clinically appropriate.
• Educate residents, families, and caregivers regarding mobility, safety, exercise programs, and functional activities.
• Complete accurate and timely documentation of evaluations, treatments, progress, and resident responses.
• Communicate resident progress and changes in condition with the interdisciplinary care team.
• Collaborate with nursing, occupational therapy, speech therapy, physicians, and other rehabilitation professionals.
• Participate in care plan meetings and discharge planning as appropriate.
• Maintain compliance with federal, state, facility, and professional standards.
• Adapt effectively to the needs, workflow, and census of each assigned facility.
• Provide consistent, compassionate, and resident-centered care across all locations.

Qualifications

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited Physical Therapy program.
• Current Illinois Physical Therapist license required.
• Skilled nursing, long-term care, subacute rehabilitation, or geriatric experience preferred.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and effectively within multiple interdisciplinary teams.
• Flexible and dependable with the ability to travel between assigned locations.
• Compassionate, professional, and resident-focused approach to care.

Position Details

• Position Type: Full-Time Float
• Locations Supported: Wood River, Godfrey & Alton, Illinois
• Setting: Skilled Nursing & Rehabilitation
• Schedule: Full-time; schedule may vary based on facility needs and census
• Travel: Local travel between assigned Nexus facilities is required
