Territory: Solano County: Petaluma* (Sonoma Country) What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST) In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team. Knowledge of state and federal homecare regulatory guidelines preferred. Experience in home health care highly preferred. Experience with high acuity patients is a plus.Experience working with an interdisciplinary team highly desired. Case Management fundamentals highly desired. The results of the assessment are confidential and will be used as one of multiple data points included in the hiring decision process. $110,000 - $125,000 a year .
