Key Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and physician orders.
- Provide skilled occupational therapy services in accordance with established care plans.
- Assess functional abilities and implement appropriate therapeutic interventions to promote independence.
- Monitor resident progress and adjust treatment approaches as clinically appropriate.
- Complete accurate and timely documentation, including evaluations, treatment notes, progress reports, and discharge summaries.
- Collaborate with nursing, therapy, and interdisciplinary team members to support positive resident outcomes.
- Participate in care planning, utilization review meetings, interdisciplinary conferences, and other clinical meetings as assigned.
- Educate residents, families, and staff regarding therapy goals, treatment plans, safety, and strategies that promote independence.
- Maintain established productivity, documentation, and quality standards.
- Support regulatory compliance, reimbursement requirements, and survey readiness.
- Promote resident safety, dignity, independence, and overall well-being.
- Graduate of an accredited Occupational Therapy program.
- Current and active state license as an Occupational Therapist.
- Long-term care or skilled nursing experience preferred.
- Strong clinical documentation and communication skills.
- Excellent interpersonal, organizational, and time-management skills.
- Ability to work effectively within an interdisciplinary healthcare team.
- Compassionate, patient-focused, and committed to providing high-quality care.
- Ability to stand, walk, bend, and assist residents throughout the workday.
- Ability to safely lift, transfer, and support residents during therapy activities.
- Ability to perform physical tasks associated with patient care and therapeutic treatment.
- Comfortable working in a clinical and long-term care environment.
- Ability to manage the physical and emotional demands of patient care.
