Licensed Clinical Social Worker (LCSW) - Riverhead, New York
Job Summary:
We are seeking a highly skilled and compassionate Licensed Clinical Social Worker (LCSW) to join our team at CHE Behavioral, a multispecialty group of licensed professionals dedicated to increasing access to mental health services. As an LCSW with CHE Behavioral, you will have the opportunity to work with a diverse patient population, providing high-quality clinical services to improve the quality of life for older adults in long-term care and rehabilitation centers.
Responsibilities:
Provide licensed clinical social work services to patients in a long-term care and rehabilitation setting, including assessment, diagnosis, treatment, and counseling
Develop and implement individualized treatment plans to meet the unique needs of each patient
Collaborate with interdisciplinary teams, including psychiatrists, psychologists, and other healthcare professionals, to ensure comprehensive care
Conduct individual and group therapy sessions, as well as family therapy as needed
Monitor patient progress and adjust treatment plans accordingly
Maintain accurate and thorough records of patient interactions and treatment plans
Stay current with industry developments and best practices in mental health care

Requirements:
Licensed Clinical Social Worker (LCSW) certification
Master's degree in Social Work (MSW) from an accredited institution
