An opportunity is available for a clinical pharmacist within a dynamic large health system known for its extensive network of ambulatory care and outpatient clinics, as well as several hospitals. Job Type: Contract (long-term, with potential for permanent placement)

Responsibilities:

• Deliver pharmaceutical care across inpatient and outpatient settings within the health system.
• Collaborate with healthcare teams to optimize medication therapy for improved patient outcomes.
• Participate in clinical rounds, evaluate patient data, and provide drug therapy recommendations.
• Support ambulatory care clinics through medication management and patient education.

Qualifications:

• Completion of a PGY1 residency strongly preferred, or approximately 2 years of recent inpatient clinical pharmacy experience.
• Demonstrated ability to work effectively in fast-paced healthcare environments.
• Strong communication skills to liaise with multidisciplinary care teams.
• Commitment to advancing patient care through evidence-based practice.
