About the Role:

As a Physical Therapist (PT) at Astoria Skilled Nursing and Rehabilitation, you will have the opportunity to make a positive impact on the lives of our guests. You will work closely with a multidisciplinary team to develop and implement individualized treatment plans designed to promote recovery, restore function, and improve overall well-being. Our rehab-recovery suites are equipped with the latest technology and equipment, providing a comfortable and inviting environment for our guests.

Responsibilities:

• Evaluate and assess patients' physical impairments, functional limitations, and disabilities

• Develop and implement individualized treatment plans to achieve patients' goals and optimize outcomes

• Provide physical therapy interventions, including exercises, modalities, and other therapies

• Collaborate with other healthcare professionals to ensure a comprehensive approach to patient care

• Conduct patient education and training to ensure patients and families understand the treatment plan and how to maintain progress after discharge

• Document patient progress, treatment plans, and outcomes in accordance with company policies and regulatory requirements

Ideal Candidate:

• Bachelor's or Master's degree in Physical Therapy

• Current licensure as a Physical Therapist (PT) in the state of Ohio
