Population Health Nurse
Position: Population Health Nurse Job ID: 2026-012 Department: Rural Health Clinic Date Posted: 3-5-2026 FTE: 1.0 Union: NON Salary Range: $70,500 to $104,00 Exempt Benefit Summary: Medical, Dental & Vision coverage options with applicable HSA & FSA. PTO Package; 401 K with company match. Life & Long-Term Disability.
Job Summary: The Population Health Nurse under the direction of the Clinic Nurse Supervisor and/or the Director of Clinic Operations will work with advanced clinical knowledge and use consistent and independent discretion and judgement to provide: Patient Education; Recommendations based on current research; Evaluation of lab test results and apply protocols to adjust medications as necessary; Liaison between chronic care patients and their PCP/Specialists; Evaluation of patient needs, plan of care with goals and progress.
Minimum Qualifications:
- MN RN Licensure
- BLS Certification
- Associate's degree in nursing
- Minimum of one-year, diabetic Ed and Anticoagulation Mgmt.
Preferred Qualifications:
- Diabetic and CCM Certification
- Bachelor of Science in Nursing
Required Education: Associates or better in Nursing.
