Work Environment

In our outpatient office located in the heart of Rome, NY, you will work alongside a collaborative team of healthcare professionals. The typical work schedule is Monday through Friday from 9:00 a.m. to 6:00 p.m., with the possibility of occasional evening clinics extending to 8:00 p.m. Participation in community outreach, health fairs, and weekend events may also be required as part of our commitment to enhancing public health awareness. This role averages approximately 40 hours per week, ensuring you have a balanced work-life schedule.

Key Responsibilities

• Conduct thorough health assessments and screenings to identify the unique needs of families and individuals in the community.
• Diagnose and manage various acute and chronic medical conditions through evidence-based treatment plans.
• Implement preventive care strategies and health promotion initiatives to optimize patient health outcomes.
• Collaborate with community organizations to create and roll out educational health programs that inform and empower patients.
• Provide culturally competent care, ensuring that all patients' rights are respected and their healthcare access is facilitated.
• Coordinate referrals and develop follow-up care plans to assure continuity in patient care.
• Maintain accurate and detailed medical documentation in line with regulatory requirements.

Qualifications

• Current certification as a Family Nurse Practitioner.
• Active state nursing license to practice in New York.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 to 200,000 annually, commensurate with your experience and qualifications.
