We are seeking a compassionate and knowledgeable professional to join our team as a Home Health Clinical Support RN. This role is critical in offering exemplary patient care at home, ensuring patients receive thorough and attentive medical support. Core Responsibilities Provide clinical support and guidance to patients and families in a home health setting Perform comprehensive patient assessments and create personalized care plans Collaborate with a multidisciplinary healthcare team to ensure comprehensive care Monitor, evaluate, and document patient progress, adjusting care plans as necessary Educate patients and caregivers on health management and medical protocols Ensure adherence to health regulations and organizational policies Work Schedule Monday through Friday, day shifts; no required weekends, on‑call, or holidays Coverage area: Glenview, Evanston, Skokie, Niles, Chicago, Park Ridge & surrounding Required Qualifications Current RN license in state of practice Minimum of 2 years of clinical experience, preferably in home health Experience with Electronic Medical Records (EMR) systems, including TruChart EMR Strong communication and interpersonal skills Ability to work independently and make informed clinical decisions Exceptional organizational skills and attention to detail Proficient in medical documentation and record-keeping Why Choose Us?
