Core Responsibilities Patient Evaluation Assess patients' functional status, strength, mobility, and physical limitations. Monitor patient progress and adjust treatment plans as needed to maximize recovery and independence. Promote patient engagement in the rehabilitation process. Documentation Accurately document evaluations, treatment plans, progress notes, and patient outcomes. Ensure compliance with facility policies and regulatory requirements. Team Collaboration Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate care and support successful patient outcomes. Assist with care transitions and discharge planning as appropriate.
