Medical Social Worker (MSW) Hospice

Schedule: Full-Time, MondayFriday
Location: Citrus Heights+ 20-mile radius

Position Overview:

Sonder Healthcare is seeking a compassionate and dedicated Medical Social Worker (MSW) to join our hospice team in a full-time, MondayFriday position. This role provides in-home psychosocial support to patients facing life-limiting illnesses and offers guidance, advocacy, and emotional support to their families.

As an essential member of our interdisciplinary hospice team, you will help patients and caregivers navigate the emotional, social, and practical challenges of end-of-life care with dignity, respect, and compassion.

Key Responsibilities:

• Conduct comprehensive psychosocial assessments and develop individualized plans of care in collaboration with the hospice team.
• Provide counseling and supportive interventions to patients and families coping with terminal illness, anticipatory grief, caregiver stress, and bereavement-related concerns.
• Assist with advance care planning and facilitate discussions regarding goals of care and end-of-life decisions.
• Collaborate with nurses, physicians, chaplains, and other interdisciplinary team members to ensure holistic, patient-centered care.
• Assess social determinants of health and address barriers impacting quality of life and care delivery.
• Provide referrals and connect patients and families to appropriate community resources and supportive services.
• Maintain accurate, timely documentation in compliance with Medicare Conditions of Participation and regulatory requirements.

Qualifications:

• Masters or Doctoral degree in Social Work from a CSWE-accredited program.
• Meets state and federal hospice/home health personnel requirements.
• Minimum of one (1) year of medical social work experience; hospice experience strongly preferred.
• Strong understanding of end-of-life care, grief processes, and family systems.
• Excellent communication, documentation, and organizational skills.
• Current CPR certification (American Heart Association).
• Valid drivers license, reliable transportation, and proof of auto insurance.
