Position Overview

We are seeking a qualified individual to join our dynamic team in an acute care facility. This role involves case management and discharge planning, with a strong focus on patient care and coordination.

Job Details

• Shift: Day 5x8-Hour (08:00 AM - 04:30 PM)
• Start Date: ASAP
• Floating Expectation: No
• Weekend Requirement: Every other weekend; must be available for holidays
• Patient Ratio: 1:20
• Years of Experience Required: 2 years in acute care case management/discharge planning
• Location: City, State

Qualifications

• Active CA State License (Pending license accepted but must be licensed upon start)
• Must have prior charge experience in a healthcare setting
• Proficiency in Epic charting system
• Experience with InterQual criteria preferred
• Knowledge in areas such as admission criteria, care coordination, discharge planning, and managing patient needs
• Understanding of insurance processes and guidelines (e.g., Medicare, Medi-Cal)

Key Responsibilities

• Coordinate patient care and manage discharges effectively
• Participate in family meetings and development of treatment plans
• Assess patients to determine medical necessity according to evidence-based guidelines
• Arrange DME, skilled nursing facilities, and ambulance transport as necessary
• Document all patient interactions and care plans using computerized documentation

Additional Details

• Patient Demographics: Neonates, Adolescents, Infants, Adults, Pediatrics, Geriatrics
• Daily Census: Case load up to 25 patients
• Interdisciplinary Support: IV Teams, Physical Therapy, Respiratory Services, Social Services, Rapid Response Teams, Pharmacy, and more

Expectations

The ideal candidate should have strong organizational skills, the ability to work independently, and a passion for patient-centric care. Must commit to working at least three of the five major holidays. Travelers may be expected to float to nearby sister facilities as needed.
