What You'll Do:

• Provide comprehensive primary care services for adult patients in an outpatient setting
• Evaluate, diagnose, and treat acute and chronic conditions
• Manage care transitions from hospital discharge to outpatient follow-up
• Coordinate with specialists and healthcare teams to deliver coordinated care
• Document medical encounters accurately using Epic EMR
• Support quality improvement initiatives aimed at enhancing patient outcomes
• Offer compassionate, culturally competent care to diverse, underserved populations

What You Need:

• Valid license and certification in Family Medicine or Internal Medicine
• Minimum of 3+ years outpatient experience; experience serving underserved populations preferred
• Excellent clinical judgment, communication, and organizational skills
• Ability to work effectively within a team environment
