Job Description
A Physical Therapist plays a vital role in helping patients recover and improve their physical function through individualized treatment plans. Working within a multidisciplinary clinical team, the therapist assesses patient conditions and implements rehabilitation exercises using standard physical therapy tools. Weekly travel for patient visits or training is typically required to provide comprehensive care. Home Health experience is a must.
Responsibilities
- Conduct patient assessments to evaluate physical conditions
- Develop and implement individualized treatment plans
- Guide patients through rehabilitation exercises
- Educate patients on managing their condition and preventing injury
- Monitor patient progress and adjust treatments accordingly
- Document treatment plans, progress notes, and outcomes
- Manage patient pain through therapeutic techniques
Preferred Qualifications
- Entry-level experience in physical therapy
- Bachelor's degree in Physical Therapy or related field
- Licensed Physical Therapist (state-specific)
- Proficient in patient assessment and treatment planning
- Knowledge of rehabilitation techniques and documentation
