Pay: $2,546.40 per week


Shifts: M-F 8am-5pm, (On call every 4th weekend and 1-2 days per week)

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members.
• Collaborate with primary care providers for comprehensive care plans.
• Reassess cost-efficient, appropriate levels of care.
• Implement non-medical support for treatment compliance.
• Coordinate care transitions and engage resources as needed.
• Maintain records of clinical, functional, and fiscal outcomes.
• Identify opportunities for health promotion and illness prevention.
• Act as a patient advocate for privacy and confidentiality.

Requirements of the RN - Case Management

• Associates or Bachelor's Degree in Nursing or related field.
• Current, unrestricted KY RN License.
• Ability to analyze complex medical information and make rational decisions.
• Case Management certification.
