Job Summary

Kirkland Court Health and Rehabilitation Center is seeking a compassionate, organized, and dedicated Director of Social Services to join our Long-Term Care (LTC) team.

The Director of Social Services is responsible for providing comprehensive psychosocial support to our residents and their families while ensuring compliance with federal and state regulations governing long-term care facilities. This position works closely with nursing, dietary, therapy, activities, administration, and other members of the interdisciplinary team to promote each resident's quality of life, dignity, rights, and overall well-being.

Responsibilities

• Provide social services and psychosocial support to residents in the LTC setting.

• Complete required social services assessments and documentation in accordance with regulatory requirements and facility policy.

• Participate in the development, implementation, and ongoing review of individualized care plans.

• Participate in interdisciplinary care plan meetings and communicate resident needs, concerns, and preferences to the appropriate team members.

• Conduct and document required social services visits and follow-up.

• Assist residents and families with adjustment to the facility and changes in condition or level of care.

• Develop and coordinate discharge plans, including communication with residents, families, hospitals, physicians, community resources, and other providers.

• Assist with admissions, transfers, and discharges as needed.

• Maintain awareness of resident rights and advocate for residents' choices, preferences, dignity, privacy, and independence.

• Respond to resident and family concerns, grievances, and complaints and ensure appropriate follow-up and documentation.

• Assist with abuse, neglect, exploitation, and misappropriation concerns in accordance with facility policy and regulatory requirements.

• Assist residents with advance directives, responsible-party concerns, and other psychosocial needs.

• Maintain appropriate communication with families and responsible parties.

• Coordinate with outside agencies and community resources when needed to meet resident needs.

• Monitor residents for psychosocial changes and communicate significant concerns to the interdisciplinary team.

• Maintain accurate, timely, and complete documentation in the electronic medical record.

• Ensure Social Services documentation and practices remain compliant with CMS, Texas Health and Human Services (HHSC), and applicable federal and state long-term care regulations .

• Assist with survey readiness and provide requested documentation during regulatory surveys.

• Protect resident confidentiality and maintain professional boundaries at all times.

• Promote a person-centered approach to care and support a positive resident experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred.

• Previous experience in long-term care, skilled nursing, healthcare, or social services is preferred but not required .

• Must demonstrate compassion, professionalism, organization, and strong communication skills.

• Must be able to work effectively with residents, families, and an interdisciplinary team.

• Must be able to maintain accurate and timely documentation.

• Knowledge of federal and Texas long-term care regulations is preferred.

• Must be able to maintain confidentiality and protect resident privacy.

• Must be able to handle sensitive situations with professionalism and discretion.

Part-Time
