Nurse Practitioner (NP)DaVita Integrated Kidney Care (IKC) is looking for a Full-Time Nurse Practitioner (NP) to join us in Scranton, PA; Wilkes-Barre, PA; Pittston, PA - must live locally.As an IKC Nurse Practitioner (NP) you will support some of our most complex patients, assisting them in navigating a challenging healthcare system. You'll focus on delivering holistic, integrated care to improve the quality of life for our kidney patients.Position DetailsLocation: Hybrid-Remote. Work from home (telehealth) and Travel within an assigned geography including Dialysis Clinics and Nephrology Practices.Performs enhanced annual wellness visits or Comprehensive Health Evaluations (CHEs) to identify and evaluate patients' co-morbid conditions and identify and assist with patients' needs, including but not limited to medical, social, emotional, and financial concerns through medical record review, consultation with patient, family, and healthcare team members. Manage complex patients, focused on reducing hospitalizations and improved health outcomesManages the care of CKD/ESKD patients, focusing on delaying disease progression (CKD), medical cost/utilization and preventing/reducing hospitalizations, through proactive post-discharge planning and follow-up with patients most at risk. Will leverage prescriptive authority, medication management, ordering/interpretation of labs/diagnostics, collaboration with other providers, etcMonday- Friday schedule with the ability to accommodate patient and nephrology partner's availability. Must be flexible.Full-TimeNo nights, weekends and on-call!Competitive pay and Excellent Benefit packageCEU/CME reimbursement + Paid License renewals & many other benefitsRequirementsPassion for delivering care to patients with complex, chronic illnessesMinimum of two (2) years' experience as a Nurse Practitioner (NP)Current unrestricted NP license to practice in the state of CT - License in NY or ability to get after hireCurrent Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hireCurrent Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certificationCurrent valid driver's license and insurance within the state of residenceIntermediate computer skills and proficiency in MS Word, Excel & Outlook requiredDemonstrated proficiency in clinical assessment and management of complex comorbidities, diagnosis, planning, implementation, documentation, and evaluation regarding the needs and care of chronic disease.Strong clinical judgment and confidence in autonomous decision-making while practicing within established protocols and evidence-based guidelinesHome Office, High Speed Internet and Reliable Transportation Required.Preferred QualificationsPreferred experience working in Value Based Care or Population Health including documenting and coding from a MA (Medicare Advantage) risk perspective and conducting Health Risk Assessments on high risk/chronically ill patients.5+ years' experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case ManagementManage pop.
