Job Summary:

The Social Services Director at Heartsworth Center Nursing is responsible for providing administrative and clinical leadership to the social services team, ensuring comprehensive patient care and social support services. As a key member of our interdisciplinary team, the Social Services Director will develop and implement policies, procedures, and programs that promote patient-centered care, timely discharges, and family engagement. Responsibilities:

• Provide strategic leadership and oversight to the social services team, ensuring effective communication and collaboration among staff, patients, and families

• Develop and implement policies, procedures, and programs that promote patient-centered care, timely discharges, and family engagement

• Collaborate with the interdisciplinary team to identify and address patient needs, resolving complex social and psychological issues

• Develop and manage departmental budgets, ensuring efficient resource allocation and effective use of funds

• Recruit, train, and mentor social work staff, providing coaching and guidance to ensure excellent patient care and service delivery

• Identify and pursue opportunities for quality improvement, accreditation, and regulatory compliance

• Maintain accreditation standards and regulatory requirements, ensuring compliance with state and federal regulations

Requirements:

• Leadership and management experience : Minimum of [X] years of experience in a social services setting, with a proven track record of successful leadership and management
