At Carolyn Downs and Country Doctor, health care is a right. Job Summary Seattle Roots Community Health is seeking a Behavioral Health Care Coordinator – Community Based Programs to provide patient navigation, care coordination, community outreach, and program support across multiple community-based programs. This position primarily supports three programs: STEP Clinic: A low-barrier substance use disorder treatment clinic co-located at Hepatitis Education Project. Health Care for the Homeless: A community-based program providing healthcare and outreach to individuals and families residing in Tiny House Villages, shelters, encampments, and other community settings. Reentry Program: A program supporting individuals transitioning back into the community following incarceration and assisting with connections to healthcare and other essential resources. Patients served through these programs may experience significant barriers to healthcare, including housing instability, substance use disorder, recent incarceration, lack of insurance, transportation challenges, and difficulty navigating healthcare and social service systems. The Behavioral Health Care Coordinator will serve as a consistent point of contact for many patients, helping them navigate healthcare and community resources while working closely with medical, nursing, behavioral health, and community partners. The position requires a strong commitment to harm reduction, patient advocacy, social service navigation, and providing services in a welcoming, inclusive, and nonjudgmental manner. This is a highly community-based position requiring the ability to work independently and collaboratively across multiple settings, including SRCH clinics, community organizations, jails, shelters, Tiny House Villages, encampments, and other outreach locations. Job Responsibilities Take a lead role in supporting the planning, development, implementation, and rollout of new pilot projects and program initiatives within programs. Coordinate project workflows, logistics, patient engagement, outreach, and implementation activities in collaboration with clinical, operational, and program leadership. Assist with developing workflows, processes, patient-facing materials, and other tools needed to successfully launch and support new initiatives. Coordinate communication and activities among internal teams, patients, and community partners as needed for program implementation. Track implementation activities, patient participation, outcomes, barriers, and other program information as assigned. Identify operational challenges and opportunities for improvement and communicate recommendations to program leadership. Support training and preparation of staff involved in new program initiatives, as appropriate. Help transition successful pilot projects into sustainable ongoing services by establishing consistent workflows and processes. Provide ongoing coordination and administrative support for pilot projects and new initiatives following implementation. Support additional program development activities as Community Based Programs continue to evolve and grow. Reentry Program Support individuals transitioning back into the community following incarceration. Coordinate day-of-release and other timely healthcare and addiction medicine appointments. Connect eligible patients with medication assistance resources to support timely access to medications. Assist patients with applying for or reactivating Medicaid and other insurance following release. Facilitate patient linkage to care through applicable jail/prison delegation agreements and community partnerships. Conduct onsite visits and outreach at jails, community organizations, and other locations serving individuals impacted by the criminal justice system. STEP Clinic & Substance Use Services Provide information to new and prospective patients regarding STEP's low-barrier model of substance use disorder treatment. Support patient registration, scheduling, insurance navigation, and other front-desk functions as needed. Work with clinical staff and nurse care managers to coordinate outreach for new, established, and prospective patients. Support day-to-day clinic operations, including responding to patient inquiries, greeting patients and visitors, assisting with clinic flow, and managing patient paperwork. Support patient engagement and navigation within a harm-reduction and low-barrier treatment environment. Understand and support patients receiving medications for substance use disorders. Assist with referrals and connections to Substance Use Disorder Professionals (SUDPs), behavioral health services, and other treatment and recovery resources. Support implementation and ongoing coordination of STEP pilot programs, group visits, and other new initiatives. Perform point-of-care testing or other clinic support activities when appropriately trained and within the scope of the position. Health Care for the Homeless & Community Outreach Conduct outreach at Tiny House Villages, shelters, encampments, and other community-based locations. Work collaboratively with nursing, medical, behavioral health, and outreach teams to identify and address barriers to healthcare. Assist individuals experiencing homelessness or housing instability with connecting to SRCH healthcare services and appropriate community resources. Provide navigation and follow-up to help patients access recommended healthcare and social services. Cultivate positive relationships with community organizations, outreach partners, site staff, and community members. Promote awareness of Seattle Roots Community Health services and Specialized Care programs. Community Partnerships & Program Support Engage professionally with community partners during outreach, care coordination, and patient advocacy. Serve as an ambassador for SRCH programs and cultivate collaborative relationships with community organizations. Participate in advocacy efforts, community meetings, outreach events, and program presentations in collaboration with Program Medical Directors and organizational leadership. Participate in assigned trainings, team meetings, case reviews, and program-planning activities. Help create and maintain welcoming, inclusive, culturally responsive, and nonjudgmental environments for patients. Demonstrate reliability in attendance, communication, documentation, and follow-through with patients, clinical staff, and community partners. Perform other related duties as assigned to support the needs and growth of Specialized Care programs. Qualifications Education & Training High school diploma or GED required. Minimum of two years of relevant experience working in community health, social services, patient navigation, care coordination, outreach, behavioral health, substance use services, or with communities experiencing significant barriers to healthcare; an equivalent combination of education and relevant experience may be considered. Ability to independently prioritize responsibilities, manage an assigned caseload, meet deadlines, and follow through on commitments. Ability to collaborate effectively with medical providers, nursing teams, behavioral health staff, community partners, and organizational leadership. Ability to work in a variety of community settings, including clinics, community organizations, jails, shelters, encampments, and Tiny House Villages. Ability to travel between assigned SRCH and community-based locations. Ability to successfully complete required background screening consistent with organizational requirements and applicable law. Required Certifications Washington State Department of Health Community Health Worker training/certification within one year of hire, if required by the organization for this position. Current CPR certification or ability to obtain certification within the timeframe established by SRCH. Complete all required organizational, safety, clinical-support, and program-specific training.
