QMHP Juvenile Justice Care Coordinator

Direct Care - Clinical Therapist/Social Work/Case Worker As a Juvenile Justice Care Coordinator (JJCC) for the Juvenile Justice Mentor program, you support youth involved in or at risk for guns, gangs, and other violent activities by coordinating care and building strong relationships. You will collaborate with Tarrant County Juvenile Services, schools, faith communities, and local organizations to interrupt cycles of violence and promote positive change. Your responsibilities include processing referrals, conducting assessments, developing treatment plans, and facilitating team meetings. You connect youth and families to community resources and provide skills training that support academic, social, and emotional growth. As a liaison between MHMR, TCJS, and community partners, you help youth disengage harmful behaviors and ensure they do not return to the Juvenile Justice system. On-the-job certification as a Qualified Mental Health Professional (QMHP) is provided.
