As a Caregiver (Part-time/Flexible Hour) , you will have the opportunity to make a meaningful impact in the lives of our patients and families. We are seeking a compassionate and caring individual to join our team, providing part-time and flexible hour support to our clients.

Responsibilities:

• Assist patients with daily living activities, including personal care, grooming, and light housekeeping
• Provide emotional support and companionship to patients, engaging in conversation, hobbies, and activities
• Monitor and report any changes in patient condition to the care management team
• Maintain accurate and detailed records of patient care and services provided
• Develop and maintain strong relationships with patients, families, and healthcare providers
