Substance Use Care Navigator — Mobile Crisis Response

South County Mental Health Center, Inc. is seeking a Substance Use Care Coordinator for our Mobile Response Team. The role focuses on care coordination for individuals involved in MRT crisis interventions, ensuring timely services and successful care linkages both internally and with external providers. The coordinator monitors linkage progress, coordinates with psychiatrists, therapists, and social service agencies, and prepares extensive monthly reports for DCF. #J-18808-Ljbffr southcountymentalhealthcenter
