Duties
Help
Conducts collaborative care assessment, diagnosis and management of patients in both the inpatient and outpatient settings.
Directs collaborative care assessment, diagnosis and management of patients in both the inpatient and outpatient settings.
Assesses acute and non-acute clinical problems.
Consults with physicians and other health care providers regarding patient management including evaluating and revising therapeutic plans and managing high-risk, unstable, or extremely complex patients.
Evaluates care delivery models and strategies, using concepts related to community, environmental/occupational health, and cultural and socioeconomic dimensions of health.
Educates patients and their families regarding healthcare processes and diagnostic tests; therapeutic regimen, including medication, diet, follow-up care; and methods of adapting medication and treatment to current lifestyles.
