Apex Home Health, Hospice & Home Care
Role: Occupational Therapist Responsibilities: Provide evidence-based evaluations and interventions to promote patients' functional independence in inpatient and outpatient settings. Design individualized treatment plans, coordinate with interdisciplinary providers, and educate patients and families to ensure comprehensive care. Qualifications & Skills: Strong clinical assessment and intervention skills, ability to collaborate within a multidisciplinary team, and excellent communication abilities. Knowledge of advanced technology and a commitment to patient-centered care are preferred.
