Brigham and Women's Advanced Primary Care Associates at South Huntington is an Internal Medicine practice that opened in August 2011. Our goal is to create an environment that maximally fosters healing relationships between patients and their healthcare team, while supporting an outstanding educational experience for future members of that healthcare team. The practice will test innovative models of primary care delivery. It will be continually improving in a way that is accessible to being evaluated and disseminated. The practice will be certifiable at the highest level as a Patient-Centered Medical Home.

Principal duties and responsibilities include performing duties under moderate supervision with intermediate to advanced proficiency in administrative skills, acting as a super user for scheduling, registration, and billing systems, providing training and assistance to others in these areas, utilizing knowledge of HMO's, managed care and other third party insurers, assisting with training and orienting staff as needed, providing cross coverage as needed, floating to other practice when help is needed, assisting with special projects as directed, following HIPAA guidelines for the management of patient privacy and confidentiality, participating in process improvement groups, participating in trainings in respect to care coordination, supporting patients in self-management, self-efficacy and behavior change, patient population management, and communication skills, as directed and required for the role, assisting co-workers in their duties, and participating in regular team meetings as well as ongoing continuous improvement efforts.

Qualifications include a minimum of high school diploma, GED or equivalent experience, minimum 1 year applicable work experience required, some additional training in office systems or other post high school education preferred, ambulatory care experience a plus, but not required, and primary care experience a plus, but not required.
