Community Health Guide: Field Navigator for Impact

Reema Health

Reema Health is seeking a Community Health Navigator (Community Guide) to engage members, enroll them in the care model, and build patient-centered plans. You will manage a moderate caseload, coordinate referrals, and document progress using Reema's platform. Role requires strong communication, empathy, and tech comfort; you will travel within a 25–50% range in a 30-mile community area, with occasional evenings and weekends.
