- Residential Direct Support ProfessionalMain Campus - Syracuse, NY 13224Education Level4 Year Degree## DescriptionThe Aftercare DSP will help youth and their families/discharge resource become integrated and engaged in their community as they are being discharged from Elmcrest’s Qualified Residential Treatment Program (QRTP).CFTSS (Children and Family Treatment and Support Services) offers opportunities to better meet the behavioral health needs at earlier junctures in a child/youth’s life to prevent the onset or progression of behavioral health condition. CFTSS is a set of services designed to help youth and their families gain the necessary tools to be successful in their homes and communities. This is done through goal driven supports and solution-based interventions.The Aftercare DSP will provide Community Psychiatric Support and Treatment Services (CPST) a service under the CFTSS umbrella while also meeting the Family First Prevention Services Act (FFPSA) Aftercare Services requirements. CPST services are goal-directed supports and solution-focused interventions intended to address challenges associated with a behavioral health needs. Activities provided under CPST are intended to assist the child/youth and family/caregivers to achieve stability and functional improvement in daily living, personal recovery and/or resilience, and family and interpersonal relationships in school and community integration.The Aftercare DSP is responsible for providing aftercare support and the following array of CPST services to the youth and family: Intensive Interventions (Counseling), Crisis Avoidance (Counseling), Intermediate Term Crisis Management (Counseling), Rehabilitative Psychoeducation, Strengths Based Service Planning, and Rehabilitative Supports.· Availability: Flexibility to create your own schedule depending on client availability but must have availability to provide services during after-school/evenings or weekends if needed. Must meet weekly billing minimum and work 40 hours for full-time role.· Documentation: Complete required documentation for Medicaid billing within prescribed timeframes for CFTSS. All case notes and casework contacts must be documented in OCFS CONNECTIONS database and Elmcrest’s EMR.· Supervision: Report to Aftercare Coordinator/Clinical Supervisor for regularly scheduled supervision to review cases, youth and family needs and service progress and/or barriers.**Department & Program Specific Responsibilities:**· Provision of Aftercare Services: The Aftercare DSP is responsible for oversight of the provision of aftercare services for a minimum of 6 month’s post-discharge from the QRTP setting. The Aftercare DSP is responsible for ensuring the youth and family/discharge resource’s needs are being met. The Aftercare DSP must know all services the youth/family are participating in during the aftercare timeframe and advocate for the youth and family/discharge resources needs.· Monthly Face-to-face Requirements: face-to-face mandates are as followed: at minimum there are to be two substantive face-to-face contacts with youth per month (at least one of the two contacts must be held at the youth's place of residence the other can be in the community). One substantive face-to-face contact with the family/discharge resource per month (this contact can include the youth but must be held at the youth's place of residence).· Permanency Planning Team Meetings: Responsible for holding at least two meetings regarding youth’s success/barriers post discharge from the residential setting. Must be a minimum of two Permanency Planning Team Meetings during the aftercare timeframe (one within the first month of discharge from the QRTP and one 30 days before the end of Aftercare Services).· Collaboration: The Aftercare DSP will collaborate with other members of the Permanency Planning Team to promote success. This can include various service providers such as; LDSS County Case Workers, other QRTP Agencies, Schools, Community Based Providers, etc.· Community Referrals: The Aftercare DSP will be responsible for making additional referrals in the community based on the youth and family/discharge resources needs.## Qualifications**Competencies:**o Must have good written and oral communication skills.o Must have positive attitude and engaging personality.o Computer skills, including Microsoft Word and Outlook.o Self-confidence and an ability to work with children and young adults with a variety of needs.o Effective problem-solving skills.o Must be reliable and able to work independently and understand the importance of maintaining confidentiality/communication with the multidisciplinary team.o Must be cleared by the NYS Justice Center through fingerprinting for a Criminal History Records search.o Must be cleared for work through Drug Testing.o Must be cleared by the NYS Office of Children and Family Services for instances of child abuse and/or neglect.o Valid, unrestricted Driver’s License, with reliable/safe transportation.o Acceptable clearance checks for State Central Register and Driver license as required by the program.o Successful completion of mandatory training.**Required Education and Experience:**o Bachelor’s degree and 2-3 years of applicable experience in children's mental health, addiction, foster care/child welfare/juvenile justice, and/or a related human services fieldORo Master’s degree (Social Work, Marriage and Family Therapy, Psychology, Mental Health Counseling, etc.)**Additional Eligibility Qualifications:**o Certification in an Evidenced Based Practice is a plus
