RN Care Manager I-IPCM - Acute Care Advocate
Mercy
Mercy is seeking a Care Manager to join the interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe transitions after hospitalization.
You will collaborate with nurses, physicians, and case managers to maintain patient experience, safety, and quality of care, using performance metrics and best practices to guide discharge planning and LOS oversight.
