Hospital-Based Community Health Case Manager
Tulsa Health Department
Tulsa Health Department seeks a dedicated Case Manager to bridge Center for Community Health clients with community health services. You will coordinate referrals, document interactions electronically, and work closely with SFHS and Laureate teams to improve patient care and outcomes. The role focuses on linking high-risk, uninsured or underinsured clients to primary care, addressing transportation, housing, PHI, and access to medications while delivering outreach in hospital and community #J-18808-Ljbffr Tulsa Health Department
