- Provide skilled nursing visits to hospice patients in their homes or facilities
- Monitor patient condition and report changes to the RN Case Manager and physician
- Administer medications and treatments as ordered
- Educate patients and families on hospice care and comfort measures
- Maintain accurate and timely clinical documentation
- Collaborate with the interdisciplinary hospice team to ensure quality patient care
- Support patients and families with compassion, dignity, and respect
- Current and active Texas LVN license
- Hospice or home health experience preferred
- Strong communication and organizational skills
- Ability to work independently and manage time effectively
- Reliable transportation and valid drivers license
- Current CPR certification
- Flexible PRN schedule
- Supportive team environment
- Meaningful work making a difference in patients lives
