Description:
Orchard Brooke Assisted Living, part of Culture Care Senior Living, is seeking a strong, compassionate Registered Nurse (RN) to support both our Assisted Living Program (ALP) and Home Care (LHCSA) services.

This is not a typical floor role. This is a clinical leadership and oversight position focused on quality care, resident outcomes, and supporting multiple areas when needed.

What You’ll Do:

Support clinical oversight in assisted living (ALP/SNALR)
Complete and review UAS assessments
Assist with care plans, case management, and high-risk residents
Support home care (LHCSA)cases and coordination
Help ensure DOH compliance and survey readiness
Step in across buildings when support is needed

Qualifications:

Active RN license (New York State)
Experience in assisted living, home care, or long-term care preferred
Knowledge of UAS, ALP, or case management strongly preferred
Strong clinical judgment and organizational skills
Team-oriented with a passion for senior care
