Job Title Physical Therapist Location Minnesota Schedule / Job Type Contract | Days | Outpatient | Onsite Key Responsibilities Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Delegate and direct all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned Adhere to discipline-specific State Practice Act and all applicable regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements Actively participate in performance improvement initiatives and contribute feedback through staff meetings, focus groups, or direct supervision channels Required Qualifications Active and unrestricted Physical Therapist licensure in the state of Minnesota Orthopedic physical therapy experience required Prior clinical experience required; new graduates will not be considered Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines Demonstrated commitment to patient safety and adherence to safe patient handling practices Preferred Experience / Skills Experience in an outpatient orthopedic setting Experience supervising or mentoring physical therapy students Active involvement in professional development activities or physical therapy organizations Why Join Us Work in a collaborative outpatient environment with a strong emphasis on quality care, patient safety, and professional growth Access to continuing education support and opportunities for individualized professional development planning Concentric Healthcare Staffing Job ID 774757.
