Ditmas Park Nursing & Rehab – Licensed Practical Nurse (LPN) Job Title: Licensed Practical Nurse (LPN) Job Location: Brooklyn, New York Company: Ditmas Park Nursing & Rehab Organization: Ditmas Park Nursing & Rehab Job Summary: We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to join our dedicated team at Ditmas Park Nursing & Rehab. As a Licensed Practical Nurse, you will provide basic medical attention to patients and assess their wellbeing as part of a larger medical team. Your duties will include checking vital signs, recording a patient's medical history, and assisting patients with hygiene. Responsibilities Administer medications and nursing procedures to patients as prescribed. Obtain and record patient temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures. Observe, monitor, and record patient symptoms and conditions to obtain and provide information for diagnosis and treatment; identify responses to medication or changes in health status, and reports status to a registered nurse, physician, or other licensed healthcare provider. Prepare patient for or assist with examinations and treatments. Maintain charts and other records of patient conditions and/or administered treatment. Educate and counsel patients regarding their health, medications, and treatment goals and procedures, as appropriate. Monitor supply inventories, and complete requisition forms to ensure adequate supply levels. As necessary, observe and supervise the activities of unlicensed direct care staff, including the administration of medication, to ensure compliance with applicable requirements and report observations to higher-level staff. Education / Certification Currently registered in New York State or has possession of a valid permit to practice as a Licensed Practical Nurse. Experience Previous long-term experience in LPN role is preferable. Good investigative skills. Ability to write, educate, and work independently. Job Type: Full-time Schedule: 8 hour shift, Monday to Friday and every other weekend. Work Location: One location, Brooklyn, New York.
