RN Case Manager Per Diem

About This Role As a Per Diem RN Case Manager at Cleveland Clinic Weston Hospital, you will work collaboratively with interprofessional teams to facilitate effective transitional care coordination. This role is ideal for experienced nurses who are passionate about patient advocacy and holistic care. Highlights Location: Weston, FL Facility: Weston Hospital Schedule: Per Diem, Day shift What You'll Do Ensure compliance with standards of accrediting bodies and state mandates. Promote interprofessional synchronization throughout the acute episode. Complete effective handoffs to the next level of care or community. Proactively assess and mitigate risks to patients’ transitions of care. Work with patients and care partners to develop individualized transitional care plans. Participate and lead in patient rounding activities, huddles, and committee meetings. Requirements Education: Graduate from an accredited school of Professional Nursing Bachelor’s of Science in Nursing (BSN) preferred Experience: Three years of recent RN clinical experience Two years of recent experience in utilization review, quality, or care management preferred Licenses & Certifications: Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification required Certification by an approved entity within one year of eligibility preferred (CTM, ACM, CCM, or AHA) About Cleveland Clinic Cleveland Clinic is a leading healthcare provider committed to delivering exceptional patient care.
