Home Health Registered Nurse

As a Home Health Registered Nurse with Orion Home Health, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our mission is to provide compassionate and personalized healthcare services to our patients, and we're looking for dedicated professionals like you to join our team.
About the Role:
As a Home Health Registered Nurse, you will:
Provide skilled nursing care to patients with various medical conditions, including wound care, medication management, and symptom control
Collaborate with our interdisciplinary team of healthcare professionals to develop and implement personalized care plans
Conduct initial and ongoing assessments to evaluate patient needs and progress
Educate patients and their families on disease management, self-care, and healthy living habits
Document patient information accurately and thoroughly in our electronic medical records system
Responsibilities:
Travel to patient homes in Oak Forest, IL and surrounding areas
Perform comprehensive assessments of patients' physical, emotional, and social needs
Develop and implement individualized care plans to achieve optimal patient outcomes
Collaborate with patients' primary care physicians and other healthcare professionals to ensure continuity of care
Provide emotional support and counseling to patients and their families
Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination

Requirements:
Minimum Years of Experience : 2 years of experience as a Registered Nurse (RN)
