The Family Nurse Practitioner will deliver comprehensive primary care services to patients of all ages, focusing on prevention, wellness, and management of acute and chronic conditions.

Essential Position Responsibilities

• Provide comprehensive primary care services, including preventive, acute, and chronic disease management for patients of all ages.
• Conduct physical exams, order and interpret diagnostic tests, and manage treatment plans.
• Educate and counsel patients on disease prevention, health maintenance, and lifestyle management.
• Collaborate with physicians, specialists, and other members of the healthcare team to ensure coordinated, patient-centered care.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in care coordination, case management, and quality improvement initiatives.

Qualifications

• Education: Graduate of an accredited Nurse Practitioner program (MSN or DNP).
• Licensure & Certification: Active or eligible Family Nurse Practitioner (FNP) license in New York; national certification required.
• Experience: Prior experience in primary care or family medicine preferred, but new graduates are encouraged to apply.
