Licensed Practical Nurse (LPN) Position

Duties include but are not limited to:

• Screens new consult for community care eligibility, specialty service requested, duplication and appropriateness of referring patients to community provider.
• Process community care referral to completion and perform timely follow up as indicated.
• Knowledge and skills of community care policies and procedures and referral pathways for community resources.
• Collaborates, communicates, sets priorities, and organize assigned work to meet deadlines, ensuring compliance with established processes, policies and regulations.
• Provide care support which will include activities of daily living, maintain a safe, clean therapeutic environment for patients while following all safety policies and procedures, assists with planning and participate in health care plan, and considers the needs of the individual patient for care including medication and equipment-assisted care.
• Provides direct nursing care to adult and geriatric patients with complex problems according to nursing standards and Medical Center policy and procedure. This includes, but is not limited to:
• Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and /or intravenous) as directed without error that may cause harm to patients. Demonstrates advance knowledge and skill in preparation, administration, and documentation of commonly prescribed oral, topical, subcutaneous, and intramuscular medications.
• Provides effective education to patients and or family members in relation to common disease processes, medication and prescribed treatment regimes.
• Demonstrates knowledge, skill, and ability to promptly recognize and effectively intervene in potentially urgent or emergent situations including hypo/hyperglycemia, acute shortness of breath, acute chest pain, seizures, stroke symptoms, mental health issues, etc.
• Schedules veteran appointments with community providers using software scheduling options and established business rules when scheduling appointments at non-VA clinics.
• Maintains advanced knowledge of VHA Scheduling Directives 1230 and VHA Consult Process Directive 1232 to include scheduling outpatient appointments, coordination of emergency, urgent and routine care, no-shows and patient cancellations.
• Participates in self-development and continuing education activities
• Communicates calmly, courteously, and professionally with all patients, personnel, and visitors while maintaining confidentiality in accordance with HIPAA regulations and Medical Center policies.

Work Schedule: Monday - Fridays, TODs 0700-1530, 0730-1600, 0800-1630
