Hill Correctional Facility Salary $54,000.00 Annually Shift Full Time Contract until 09/30/2027 Summary The Program Coordinator will be responsible for coordinating the workflow of the Substance Use Disorder (SUD) reentry program, including coordinating screening, assessments, assigning patients to the Reentry Planner, tracking program data, evaluating outcomes, and completing timely quarterly reports to DOJ. The Coordinator will also be responsible for stakeholder outreach and engagement to support strategic and data-driven decision-making both within the public safety and criminal justice system and the local network of behavioral health care providers. Examples of Duties (Illustrative Only) Implement and Coordinate the workflow of the SUD reentry grant. Maintain a reentry tracker for program data to ensure the accurate and reliable tracking of individuals as they move through the different phases of the reentry process to include incarceration, prerelease treatment and planning, release, post release case management and support services. Effectively monitor progress, track outcomes, and evaluate the effectiveness of interventions and services provided. Complete timely monthly, quarterly, yearly, and close out reports. Responsible for stakeholder outreach and engagement to support strategic data driven decision making within the public safety and criminal justice system and local behavioral health care providers. Responsible for the reporting mechanisms within the existing electronic records management system. Coordinate with local law enforcement, Delaware County Probation and Parole Departments and the Delaware County Court of Common Pleas to identify the number and percentage of offenders who have violated their community supervision, been rearrested, been reconvicted and or returned to incarceration. Track this information for 12 months after initial release. Maintain annual data to track improvements in outcomes from the baseline recidivism data collected at the start of the project. Work with Reentry Planner to collect data on reentry client outcomes including housing and employment status, engagement in mental health services, engagement in Peer/recovery support services and engagement in SUD treatment for six months post release. Convene quarterly stakeholder meetings to increase communication and coordination of services, ensure cross sector collaboration between public health and safety professionals, strengthen our network of partnering service providers and identify and address gaps or limitations in services using program outcomes data. Facilitate frequent focus groups to solicit input and feedback from justice-involved individuals/individuals with lived experiences. Other duties as assigned. Required Knowledge, Skills, and Competencies Bachelor’s degree in criminal justice, sociology, or a closely related field is required. Master’s degree in criminal justice, social work or a related field from an accredited college or university; or a closely related field is desirable. One (1) year experience in data tracking, community engagement and/or grant reporting. Two (2) Prior experience in social work and/or case management with justice involved individual or individual with lived experience. Possess or develop a thorough understanding of the Principles of Effective Intervention that empirical evidence demonstrates reduces recidivism. Must possess the ability to work with a team of professionals and implement team strategy approaches to work assignments. Must be flexible, cooperative, and empathetic to needs of staff and incarcerated persons. Ability to prepare written reports; to communicate effectively, both orally and in writing; and to learn rules, regulations, and security procedures required by the facility. Ability to work with computers and the necessary software typically used by the department. Licenses, Registrations, Certifications, or Special Requirements: N/A Standard jail facility and office environment. Incumbent performs duties in a jail facility and involve sitting and walking at will, walking/standing and/or sitting for long periods, lifting/ carrying heavy objects, crouching/kneeling, bending at the waist, close and far vision, reaching, handling/grasping/fingering objects, hearing sounds/communication. Incumbent is frequently exposed to the hazards associated with jail operations, such as potentially violent individuals and communicable disease. No prolonged extreme physical demands are associated with normal duties or assignments.
