Nurse Practitioner Primary Care Cleveland Ohio

HarmonyCares is seeking a Nurse Practitioner to deliver home-based primary care to medically complex, home-limited adults in the Cleveland, OH area. In this role, you will perform in-home and telehealth visits, complete comprehensive assessments, manage chronic and acute conditions, and coordinate care with an interdisciplinary team. You will order and interpret diagnostics, prescribe medications, and educate patients and caregivers to improve outcomes and reduce avoidable hospitalizations. Working within a mission-driven, patient-first culture, you will document in the EHR, support quality initiatives, and play a key role in expanding access to compassionate primary care for vulnerable patients.
