Full-Time Occupational Therapist Opportunity

Continuum Therapy Partners is seeking a Licensed Occupational Therapist (OT) to join our rehabilitation team serving Morningside Health Center and Sheboygan Progressive Center in Sheboygan, Wisconsin . This full-time opportunity allows you to provide occupational therapy services in skilled nursing facilities while helping adult and geriatric patients regain independence and improve their quality of life.

Occupational Therapist (OT) Responsibilities

• Evaluate adult and geriatric patients and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services focused on improving activities of daily living (ADLs), upper extremity function, mobility, cognition, and functional independence.
• Complete patient evaluations, progress notes, reassessments, and discharge documentation.
• Collaborate with Physical Therapists, Physical Therapist Assistants, Speech-Language Pathologists, nursing staff, physicians, and caregivers.
• Educate patients and families on adaptive equipment, home safety, and therapeutic exercises.
• Maintain accurate documentation in accordance with Medicare, state, federal, and company guidelines.
• Participate as an active member of the interdisciplinary rehabilitation team.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Wisconsin Occupational Therapist (OT) license required.
• Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or outpatient settings is preferred but not required.
• New graduate Occupational Therapists are encouraged to apply.
• Strong communication, clinical reasoning, and teamwork skills.
