What You'll Do:

• Provide outpatient and inpatient physical therapy services to diverse patient populations
• Assess patient needs and develop personalized treatment plans
• Educate patients and caregivers on home care strategies
• Document clinical interventions accurately in EMR (Meditech Home Care)
• Collaborate with interdisciplinary team members to ensure optimal patient outcomes
• Support within the department as an educator and team contributor
• Travel throughout a 50-mile radius serving various locations

What You Need:

• Valid state licensure as a Physical Therapist
• BLS certification required
• Minimum 1 year of acute care experience
• Strong clinical skills in general medical and surgical therapies
• Ability to work independently and as part of a team
• Preference for experience in home health or outpatient settings
