Position: FACT Provider (Advanced Practice Provider) Exemption Status: Exempt Reporting Relationship: Chief Medical Officer Work Environment: Community-based outpatient program combining clinic-based medication management with field visits to consumers' homes; local travel within the Oklahoma City metro area is required (travel compensated)
Position Summary
The FACT Provider is an Advanced Practice Provider recognized as a Psych Certified APRN by the Board of Nursing or a Certified Physician Assistant by the Board of Medicine. This role delivers diagnostic evaluation and pharmacological management services for consumers enrolled in NorthCare's Flexible Assertive Community Treatment (FACT) program, a community-based interdisciplinary model serving adults with severe mental illness whose needs have not been effectively met through office-based treatment alone. Priority is given to consumers with psychosis and a history of multiple hospitalizations. Working alongside RNs, therapists, and rehabilitation specialists, the FACT Provider delivers evaluation, medication management, and crisis support both on-site and in the community, including in consumers' homes, as part of an integrated, recovery-oriented treatment team.
Essential Duties and Responsibilities
- Provides psychiatric diagnostic evaluations of consumers, including collection and documentation of an adequate history, mental status exam, assessment, and plan, including prescription of medications where appropriate.
- Provides ongoing evaluation and management services for consumers, assessing mental health and treatment adequacy through interval histories and mental status exams, and completing vital signs and targeted physical exams as appropriate.
- Assesses therapeutic response and side effects related to medications, adjusting the medication regimen and referring for additional services (e.g., therapy, DBT, WRAP) when indicated.
- Follows established guidelines and best practices in prescribing psychotropic medications.
- Accurately documents treatment recommendations, medications prescribed, and medication changes, with rationale for prescribing or changes.
- Monitors for and documents adverse effects, medication interactions, and impact of comorbid physical health conditions; coordinates care with primary care physicians when appropriate.
- Completes AIMS and other forms as appropriate.
- Educates consumers about their condition and medications, engaging them in shared decision-making regarding treatment options.
- Obtains and documents informed consent for medications prescribed.
- Orders appropriate laboratory tests for monitoring of treatment and screening for common medical comorbidities.
- Completes required documentation within one (1) business day of on-site services and two (2) business days of services provided in the community, per policy.
- Provides on-site, phone, or community-based crisis assessment and management for consumers as needed.
- Provides emergency "on call" services to resolve crisis situations as assigned or directed.
- May provide on-site supervision for physician extenders when the primary supervisor is unavailable, and clinical coverage for other medication clinic providers in their absence.
- Attends staff meetings and required training.
- Works as an active member of the treatment team, utilizing all available resources.
- Prepares required paperwork in an accurate and timely manner.
Qualifications
Required Qualifications
Education
- Completion of an accredited Advanced Practice Provider educational program
Experience
- Current recognition as a Psych Certified APRN or Certified Physician Assistant (for Advanced Practice Providers)
Certifications/Licensure
- Active, unrestricted license to practice medicine in the state of Oklahoma, or applicable APRN/PA licensure
- Board certification or board eligibility in psychiatry, or recognition as a Psych Certified APRN (Board of Nursing) or Certified Physician Assistant (Board of Medicine)
- Current certification with the Drug Enforcement Agency (DEA) and Oklahoma Bureau of Narcotics and Dangerous Drugs (must obtain within 30 days of hire if not current)
- Current CPR, First Aid, and CAPE certifications (must obtain within 30 days of hire if not current)
- Valid Oklahoma driver's license
- Core Level Co-Occurring Training and Trauma Informed Training via myLearningPointe (within 60 days of hire)
- Current TB test (within 12 months prior to hire or within 30 days of hire)
Technical Skills
- Ability to safely prescribe and monitor psychotropic medications, following established best-practice guidelines
- Strong diagnostic, risk-assessment, and crisis-management skills
- Proficiency with electronic health record (EHR/EMR) documentation
- Effective verbal communication skills with English-speaking consumers
Preferred Qualifications
- Prior experience in a FACT, ACT, or other community-based behavioral health treatment model
- Experience providing psychiatric services in home or community settings
Working Conditions / ADA Requirements
- Ability to travel locally within the Oklahoma City metro area to conduct home and community visits (travel compensated)
- Ability to respond to emergency "on call" crisis situations as assigned
- Prolonged sitting and computer use during clinic-based documentation and medication management
