Physical Therapist (PT) - Outpatient

Role Overview: A full-time Mobile Outpatient Physical Therapist provides patient-centered rehabilitation services primarily in in-home settings, including private residences and independent living facilities. They assess, develop, and implement individualized therapy programs to restore and enhance mobility, strength, and independence. Responsibilities include evaluating patients, designing treatment plans, instructing patients and caregivers, making referrals to other disciplines, and ensuring safety and compliance. The role involves supervising support personnel, maintaining documentation, participating in team meetings, and adapting care plans as needed. Qualifications & Skills Valid Physical Therapy license in applicable state(s) or license eligibility Master's degree in Physical Therapy or equivalent; or Bachelor's degree with 5+ years of experience Experience in outpatient, home-based, or community rehabilitation preferred Reliable transportation and valid driver's license Strong assessment, clinical, and communication skills Key Duties Conduct patient evaluations, develop treatment plans, and apply therapeutic techniques Educate and train patients and caregivers for promoting independence Design and fit adaptive devices, including orthotics and prosthetics Collaborate with healthcare team and refer to other disciplines as necessary Ensure safety, maintain equipment, and complete documentation Support health, wellness, and fall prevention initiatives #J-18808-Ljbffr Proforma Health
