Physical Therapist (PT) - Outpatient

Schedule: 8 hr days - 8am to 4pm, primarily Monday to Friday

Assignment Details: Guaranteed hours: 40 hours per week. Contract length: 13 weeks.

Roles & Responsibilities:

• Provide patient-centered care to an average of 14 patients per day.
• Implement treatment plans and assess patient progress.
• Collaborate with interdisciplinary teams to coordinate care.
• Educate patients and families on health management and resources.
• Maintain accurate and timely documentation in patient records.
• Adhere to all regulatory guidelines and facility policies.

Compliance / Notes: Orientation details and compliance with facility protocols will be provided.
