Nurse Practitioner for In-home Visits

Overview Signify Health, part of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on overall health and well‑being to identify barriers to care. Perform medication and medical history reviews and physical evaluations. Complete patient‑specific forms and adhere to assessment protocols without prescribing medication, ordering lab tests, or altering treatment regimens. Compensation & Benefits $80 base rate per completed assessment (average 6-9 visits daily). Earn up to $2,500–$4,000 each week. Increase per‑visit rate up to 135% or 160% using Market Multiplier (varies by county). Additional pay‑per‑test compensation for qualifying visits. Availability incentives based on weekly availability. Expenses: Airfare, hotel, and rental car coverage available (depending on eligibility). Malpractice coverage provided. Independent contractor status (1099). Schedule & Contract Shifts available Monday‑Sunday; no nights or weekends required. Positions are short‑term or long‑term, with pre‑scheduled in‑home visits.
