PRN Home Care Resident Assistant

About the Role:

We're seeking a compassionate PRN Home Care Resident Assistant to provide quality physical, psycho-social, and social support to residents in their home environments. Duties include personal care, meal prep, medication reminders, errands, companionship, light housekeeping, and escorting residents to appointments.

Responsibilities:

• Deliver care per individualized care plans
• Maintain annual education and required state certifications
• Communicate changes in resident condition to the Home Care Manager
• Document care accurately and promptly
• Keep resident environments clean and follow infection control guidelines
• Escort residents to medical appointments and ensure proper paperwork and comfort throughout the visit

Qualifications:

• Completion of approved nursing assistant program + certification exam
• 1+ year of home care or long-term care experience
• Valid PA driver's license & reliable personal vehicle
• Ability to lift/transfer 50250 lbs and perform frequent standing, walking, and bending
