Actify Home Care is growing, and looking for Geriatric SLP home health therapists to join our team in South Broward County: Hollywood, Pembroke Pines, Miramar, Davie, Weston, Hallandale Beach, and surrounding areas.
SLP Per-Visit Compensation (W-2 employee)
- Evaluation / Re-Admission: $100.00
- Routine Visit: $80.00

What You’ll Do
- Provide high-quality, one-on-one speech, cognitive, and swallowing therapy to geriatric patients in their homes (focusing on dysphagia, stroke recovery, and cognitive-linguistic care). We are open to skilled clinicians looking to transition their foundational SLP experience into adult home health.
- Complete Evaluations, Re-Assessments, and OASIS documentation (Start of Care, Resumption of Care, and Discharge).
- Develop and implement plans of care to address speech, language, and Dysphagia (swallowing) disorders.
What You Need
- License: Active Florida Speech-Language Pathologist (SLP) License.
- Certification: Current CPR/BLS Certification.
- Transportation: Valid Driver’s License and reliable transportation.
- Software: Kinnser/WellSky experience is a major plus!
