Field-Based RN Case Manager - Wound Care & Infusion
Adaptive Home Health
Adaptive Home Health is seeking an RN Case Manager for a field-based role across Texas. You will own patient assessments, document visits (OASIS), and coordinate high-acuity home health care, including wound care and infusion therapy. Build care plans and communicate with physicians and families while supervising LVNs/HHAs when applicable. This full-time position uses a productivity-based compensation model (per point) with 30 points/week, supported by a clinical and operational team to enable #J-18808-Ljbffr Adaptive Home Health
